Februar 2013 Der Gemeinsame Bundesausschuss hat in seiner Sitzung am 21. Februar 2013 beschlossen, die Richtlinie über die Verordnung von Arzneimitteln in der vertragsärztlichen Versorgung (Arzneimittel-Richtlinie) in der Fassung vom 18. Dezember 2008/22. Januar 2009 (BAnz. Nr. 49a vom 31. März 2009), zuletzt geändert am T. Monat JJJJ (BAnz AT TT.MM.JJJJ V [V-Nr.]), wie folgt zu ändern: I. Rechtsgrundlage Der Gemeinsame Bundesausschuss (G-BA) hat nach § 31 Abs. 1 Satz 2 und 3 SGB V in den Richtlinien nach § 92 Abs. 1 Satz 2 Nr. 6 festzulegen, in welchen medizinisch notwendigen Fällen Stoffe und Zubereitungen aus Stoffen, die als Medizinprodukte nach § 3 Nr. 1 oder 2 des Medizinproduktegesetzes (MPG) zur Anwendung am oder im menschlichen Körper bestimmt sind, ausnahmsweise in die Arzneimittelversorgung einbezogen werden. § 34 Abs. 1 Satz 5, 7 und 8 sowie Abs. 6 und die §§ 35, 126 und 127 SGB V gelten entsprechend. Für verschreibungspflichtige und nicht verschreibungspflichtige Medizinprodukte nach § 31 Abs. 1 Satz 2 SGB V gilt § 34 Abs. 1 Satz 6 entsprechend. Nach § 31 Abs. 1 Satz 2 2. Halbsatz i. V. m. § 34 Abs. 6 SGB V können Hersteller von Medizinprodukten beim Gemeinsamen Bundesausschuss Anträge zur Aufnahme von Medizinprodukten in die Anlage V zum Abschnitt J der Arzneimittel-Richtlinie (Übersicht der verordnungsfähigen Medizinprodukte) stellen. Der Gemeinsame Bundesausschuss hat ausreichend begründete Anträge innerhalb von 90 Tagen zu bescheiden. Nach Kapitel 4 § 38 Abs. 3 der Verfahrensordnung des Gemeinsamen Bundesausschusses (VerfO) sind Änderungen der Angaben gemäß 2. Kapitel § 38 Abs. 2 Verfahrensordnung des Gemeinsamen Bundesausschusses dem Gemeinsamen Bundesausschuss unverzüglich mitzuteilen 2. Eckpunkte der Entscheidung Aufgrund einer Änderungsmitteilung des Herstellers zu dem u. g. Medizinprodukt in der Anlage V der Arzneimittel-Richtlinie hat der Gemeinsame Bundesausschuss eine entsprechende Änderung der Verordnungsvoraussetzungen in Anlage V der Arzneimittel- Richtlinie zu folgendem Medizinprodukt beschlossen. Lubricano® Steriles Gel 3. Bürokratiekosten Durch die im Beschluss enthaltenen Regelungen entstehen keine Informationspflichten für Leistungserbringerinnen und Leistungserbringer im Sinne von Anlage II zum 1. Kapitel Verfahrensordnung des Gemeinsamen Bundesausschusses. Daher entstehen auch keine Bürokratiekosten. 3 4. Verfahrensablauf Bei der Geschäftsstelle des Gemeinsamen Bundesausschusses eingehende Änderungsmittteilungen zu Medizinprodukten in der Anlage V der Arzneimittel-Richtlinie werden im Unterausschuss Arzneimittel sowie in der vom Unterausschuss eingesetzten Arbeitsgruppe Medizinprodukte beraten. Der Unterausschuss Arzneimittel empfiehlt dem Gemeinsamen Bundesausschuss, entsprechende Änderungen der Arzneimittel-Richtlinie zu beschließen. Berlin, den 21. Both the PDE approach and the ‘current approach’ should be equally acceptable. In any case it should not be a requirement for all products manufactured. The PDE-approach may particularly benefit hazardous contaminants such as highly sensitizing materials (such as beta lactams), biological preparations (e.g. from live micro- organisms), certain hormones, cytotoxics and other highly active materials. Proposed change (changes in bold): 1.6 Cross-contamination should be avoided for all products by appropriate design and operation of manufacturing facilities. The measures to prevent should be commensurate with the risks. Quality Risk Management principles should be used to assess and control the risks. This document provides guidance for the interpretation of the principles and guidelines of good manufacturing practice (GMP) for medicinal products as laid down in Directive 2003/94/EC for medicinal products for human use and Directive 91/412/EEC for veterinary use. Reasons for changes: The only change is to section 6 as part of the improved guidance on prevention of cross-contamination involving also Chapter 5 and includes reference to a new complementary toxicological assessment guidance. Telephone: (32-2) 299 11 11 ER 3 PREMISES AND EQUIPMENT Principle Premises and equipment must be located, designed, constructed, adapted and maintained to suit the operations to be carried out. Their layout and design must aim to minimise the risk of errors and permit effective cleaning and maintenance in order to avoid cross-contamination, build-up of dust or dirt and, in general, any adverse effect on the quality of products. Premises General 3.1 Premises should be situated in an environment which, when considered together with measures to protect the manufacture, presents minimal risk of causing contamination of materials or products. 3.2 Premises should be carefully maintained, ensuring that repair and maintenance operations do not present any hazard to the quality of products. They should be cleaned and, where applicable, disinfected according to detailed written procedures. 3.3 Lighting, temperature, humidity and ventilation should be appropriate and such that they do not adversely affect, directly or indirectly, either the medicinal products during their manufacture and storage, or the accurate functioning of equipment. 3.4 Premises should be designed and equipped so as to afford maximum protection against the entry of insects or other animals. 3.5 Steps should be taken in order to prevent the entry of unauthorised people. Production, storage and quality control areas should not be used as a right of way by personnel who do not work in them. Production Area 3.6 Cross- contamination should be avoided for all products by appropriate design and operation of manufacturing facilities. The measures to prevent cross-contamination should be commensurate with the risks. Quality Risk Management principles should be used to assess and control the risks. Dedicated facilities are required for manufacturing when a medicinal product presents a risk: a) Which cannot be adequately controlled by operational and/ or technical measures or b) Scientific data does not support threshold values (e.g. allergenic potential from highly sensitising materials such as beta lactams) or c) Threshold values derived from the toxicological evaluation are below the levels of detection Further guidance including some exemptions could be found in Chapter 5 and in Annex 2, 3, 4, 5 of the EU detailed guidelines on GMP and the guideline on setting health based exposure limits for use in risk identification in the manufacture of different medicinal products in shared facilities. 1 3.7 Premises should preferably be laid out in such a way as to allow the production to take place in areas connected in a logical order corresponding to the sequence of the operations and to the requisite cleanliness levels. 3.8 The adequacy of the working and in-process storage space should permit the orderly and logical positioning of equipment and materials so as to minimise the risk of confusion between different medicinal products or their components, to avoid cross-contamination and to minimise the risk of omission or wrong application of any of the manufacturing or control steps. 3.9 Where starting and primary packaging materials, intermediate or bulk products are exposed to the environment, interior surfaces (walls, floors and ceilings) should be smooth, free from cracks and open joints, and should not shed particulate matter and should permit easy and effective cleaning and, if necessary, disinfection. 3.10 Pipework, light fittings, ventilation points and other services should be designed and sited to avoid the creation of recesses which are difficult to clean. As far as possible, for maintenance purposes, they should be accessible from outside the manufacturing areas. 3.11 Drains should be of adequate size, and have trapped gullies. Open channels should be avoided where possible, but if necessary, they should be shallow to facilitate cleaning and disinfection. 3.12 Production areas should be effectively ventilated, with air control facilities (including temperature and, where necessary, humidity and filtration) appropriate both to the products handled, to the operations undertaken within them and to the external environment. 3.13 Weighing of starting materials usually should be carried out in a separate weighing room designed for such use. 3.14 In cases where dust is generated (e.g. during sampling, weighing, mixing and processing operations, packaging of dry products), specific provisions should be taken to avoid cross contamination and facilitate cleaning. 3.15 Premises for the packaging of medicinal products should be specifically designed and laid out so as to avoid mix-ups or cross-contamination. 3.16 Production areas should be well lit, particularly where visual on-line controls are carried out. 3.17 In-process controls may be carried out within the production area provided they do not carry any risk to production. Storage Areas 1 http://www.ema.europa.eu/ema/index.jsp?curl=pages/includes/document/document_detail.jsp?webContentId=W C500137091&mid=WC0b01ac058009a3dc 3.18 Storage areas should be of sufficient capacity to allow orderly storage of the various categories of materials and products: starting and packaging materials, intermediate, bulk and finished products, products in quarantine, released, rejected, returned or recalled. 3.19 Storage areas should be designed or adapted to ensure good storage conditions. In particular, they should be clean and dry and maintained within acceptable temperature limits. Where special storage conditions are required (e.g. temperature, humidity) these should be provided, checked and monitored. 3.20 Receiving and dispatch bays should protect materials and products from the weather. Reception areas should be designed and equipped to allow containers of incoming materials to be cleaned where necessary before storage. 3.21 Where quarantine status is ensured by storage in separate areas, these areas must be clearly marked and their access restricted to authorised personnel. Any system replacing the physical quarantine should give equivalent security. 3.22 There should normally be a separate sampling area for starting materials. If sampling is performed in the storage area, it should be conducted in such a way as to prevent contamination or cross-contamination. 3.23 Segregated areas should be provided for the storage of rejected, recalled or returned materials or products. 3.24 Highly active materials or products should be stored in safe and secure areas. 3.25 Printed packaging materials are considered critical to the conformity of the medicinal product and special attention should be paid to the safe and secure storage of these materials. Quality Control Areas 3.26 Normally, Quality Control laboratories should be separated from production areas. This is particularly important for laboratories for the control of biologicals, microbiologicals and radioisotopes, which should also be separated from each other. 3.27 Control laboratories should be designed to suit the operations to be carried out in them. Sufficient space should be given to avoid mix-ups and cross-contamination. There should be adequate suitable storage space for samples and records. 3.28 Separate rooms may be necessary to protect sensitive instruments from vibration, electrical interference, humidity, etc. 3.29 Special requirements are needed in laboratories handling particular substances, such as biological or radioactive samples. Ancillary Areas 3.30 Rest and refreshment rooms should be separate from other areas. 3.31 Facilities for changing clothes and for washing and toilet purposes should be easily accessible and appropriate for the number of users. Toilets should not directly communicate with production or storage areas. 3.32 Maintenance workshops should as far as possible be separated from production areas. Whenever parts and tools are stored in the production area, they should be kept in rooms or lockers reserved for that use. 3.33 Animal houses should be well isolated from other areas, with separate entrance (animal access) and air handling facilities. Equipment 3.34 Manufacturing equipment should be designed, located and maintained to suit its intended purpose. 3.35 Repair and maintenance operations should not present any hazard to the quality of the products. 3.36 Manufacturing equipment should be designed so that it can be easily and thoroughly cleaned. It should be cleaned according to detailed and written procedures and stored only in a clean and dry condition. 3.37 Washing and cleaning equipment should be chosen and used in order not to be a source of contamination. 3.38 Equipment should be installed in such a way as to prevent any risk of error or of contamination. 3.39 Production equipment should not present any hazard to products. Parts of production equipment that come into contact with the product must not be reactive, additive or absorptive to such an extent that it will affect the quality of the product and thus present any hazard. 3.40 Balances and measuring equipment of an appropriate range and precision should be available for production and control operations. 3.41 Measuring, weighing, recording and control equipment should be calibrated and checked at defined intervals by appropriate methods. Adequate records of such tests should be maintained. 3.42 Fixed pipework should be clearly labelled to indicate the contents and, where applicable, the direction of flow. 3.43 Distilled, deionised and, where appropriate, other water pipes should be sanitised according to written procedures that detail the action limits for microbiological contamination and the measures to be taken. 3.44 Defective equipment should, if possible, be removed from production and quality control areas, or at least be clearly labelled as defective.

KG Arzneimittel 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4: entfällt 7 V. Valproinsäure bei der Migräneprophylaxe im Erwachsenenalter 1. Hinweise zur Anwendung von Valproinsäure gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Migräneprophylaxe von Erwachsenen ab 18 Jahren, wenn eine Behandlung mit anderen dafür zugelassenen Arzneimitteln nicht erfolgreich war oder kontraindiziert ist. Die Verordnung darf nur durch Fachärzte für Nervenheilkunde, für Neurologie und/oder Psychiatrie oder für Psychiatrie und Psychotherapie erfolgen. b) Behandlungsziel: klinisch relevante Reduzierung der Frequenz von Migräneattacken ( ≥ 50%) c) Folgende Wirkstoffe sind zugelassen: Metoprololtartrat (Ph.Eur.) Propanololhydrochlorid Flunarizin Topiramat Dihydroergotamin(mesilat) d) Spezielle Patientengruppe: Erwachsene mit Migräne, mit oder ohne Aura, bei denen eine Migräneprophylaxe indiziert ist, wenn eine Therapie mit allen anderen dafür zugelassenen Arzneimitteln nicht erfolgreich war, wegen Nebenwirkungen abgebrochen werden musste oder wegen Kontraindikationen nicht initiiert werden konnte. Hinweise: Voraussetzung für die Behandlung von Frauen im gebärfähigen Alter ist eine sehr sorgfältige Nutzen-Risiko-Abwägung und eine umfassende Beratung durch den behandelnden Arzt, darüber hinaus müssen die Frauen durch den behandelnden Arzt über das erhöhte Risiko von Missbildungen sowie darüber aufgeklärt werden, dass während der Behandlung mit Valproinsäure eine effektive Methode der Kontrazeption erforderlich ist. Aufgrund des unter der Behandlung mit Antiepileptika beschriebenen Auftretens von Suizidgedanken und suizidalem Verhalten erfordert insbesondere die Behandlung von Patienten mit Depressionen ein sorgfältiges Monitoring. Auch bei Patienten mit einer Epilepsie oder bipolaren Störung, für deren Behandlung Valproinsäure zugelassen ist, kann eine Migräneprophylaxe erforderlich sein. Da aussagefähige Studien zu einer kombinierten Indikation („Doppelindikation“) nicht vorliegen, bedarf der Einsatz von Valproinsäure bei dieser Patientengruppe einer besonderen fallindividuellen Abwägung, insbesondere ist das Nutzen-Risiko-Verhältnis von Valproinsäure im Vergleich zu vorbestehenden oder alternativen Therapieregimen auch fachärztlich zu bewerten. e) Patienten, die nicht behandelt werden sollten: (Gegenanzeigen entsprechen denen der Fachinformation.) - Schwangere Frauen sind in jedem Fall von der Behandlung auszunehmen. - Frauen, die keine Kontrazeption vornehmen. - Patienten mit episodischen Kopfschmerzen vom Spannungstyp oder medikamenten-induzierten Kopfschmerzen. f) Dosierung: Es wird eine Monotherapie mit einer anfänglichen Tagesdosis von 500 mg/Tag 8 empfohlen, die ggf. wirkungsabhängig bis 1500 mg/Tag gesteigert werden kann. Tagesdosen über 1500 mg sind nicht ausreichend untersucht. g) Behandlungsdauer: Die therapeutische Wirksamkeit kann erst nach einer Behandlungsdauer von 3 Monaten, unter Berücksichtigung der individuellen Attackenfrequenz beurteilt werden. Hierzu ist ein Schmerztagebuch durch den Patienten zu führen. In der Regel wird eine Langzeittherapie erforderlich sein. h) Wann sollte die Behandlung abgebrochen werden? Neben den in der Fachinformation aufgeführten Gründen sollte die Behandlung abgebrochen werden, wenn das Therapieziel einer 50%igen Reduktion der Attackenfrequenz nicht erreicht wird. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: In den geprüften Studien wurde unter Ko-Therapie mit Triptanen über keine Wechselwirkungen berichtet. j) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Valproinsäure- haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: AWD.pharma GmbH & Co. Anwendung von Fludarabin bei anderen als in der Zulassung genannten niedrig bzw. intermediär malignen B - Non-Hodgkin-Lymphomen (B-NHL) als chronische lymphatische Leukämien (CLL) 1. Hinweise zur Anwendung von Fludarabin gemäß § 30 Absatz 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Fludarabin in Kombination mit Cyclophosphamid, Mitoxantron und Rituximab (R- FCM) bei geeigneten Patienten mit niedrig oder intermediär malignen Non- Hodgkin-Lymphomen der B-Zellreihe (CD20 positive NHL, u. a. lymphozytisch, lymphoplasmozytisch, lymphoplasmazytoid, follikulär Grad 1 oder 2, Mantelzell, Marginalzonen, nicht multiples Myelom, nicht Haarzellleukämie) und Resistenz auf CHOP (mit oder ohne Rituximab) b) Behandlungsziel: palliative Therapie, Verlängerung der Überlebenszeit c) Folgende Wirkstoffe sind für die Indikation NHL (ohne supportive Therapie) zugelassen: Vinca-Alkaloide: Vincristin, Vinblastin (2nd line) Anthrazykline und Analoga: Adriamycin (nur zur Therapie hochmaligner NHL), Epirubicin, Mitoxantron Podophyllotoxinderivate: Etoposid (nur intermediär und hochmaligne NHL) Folsäure-Analoga: Methotrexat Stickstoff-Lost-Analoga: Cyclophosphamid, Chlorambucil, Trofosfamid, Ifosfamid Pyrimidin-Analoga: Cytarabin Nitrosoharnstoff: Bendamustin Cytotoxische Antibiotika: Bleomycin (nur intermediär und hochmaligne NHL) Glucocorticoide: Prednisolon, Prednison Amidohydrolase: Asparaginase (nur bei pädiatrischen NHL) Anti-CD20-Antikörper: Rituximab, Ibritumomab-Tiuxetan Interferon: Interferon-alfa (nur follikuläres NHL) d) Spezielle Patientengruppe: Mit systemischer Chemotherapie vorbehandelte Patienten im Erwachsenenalter (> 18 Jahre) mit krankheitsbedingten Symptomen und/oder dem Nachweis der fortschreitenden Lymphomerkrankung e) Patienten, die nicht behandelt werden sollten: Patienten mit multiplem Myelom oder Haarzellleukämie sollten nicht behandelt werden. Es sind die in den Fachinformationen genannten Gegenanzeigen, Warnhinweise und Vorsichtsmaßnahmen zu beachten. In der Literatur konnten keine Angaben gefunden werden, dass darüber hinaus bei Patienten mit niedrig oder intermediär malignen NHL zusätzliche Einschränkungen bestehen. f) Dosierung (z. B. Mono- oder Kombinationstherapie, gleichzeitig, zeitversetzt, Abstand usw.): R-FCM-Regime (Forstpointner R. et al., 2004): 10 Fludarabin 25 mg/m2 Körperoberfläche i.v., Tag 1-3 Cyclophosphamid 200 mg/m2 Körperoberfläche i.v., Tag 1-3 Mitoxantron 8 mg/m2 Körperoberfläche i.v., Tag 1* Rituximab 375 mg/m2 Körperoberfläche i.v. Tag 0* Zykluswiederbeginn Tag 29 * Bei Kombination von Rituximab und zytostatischer Chemotherapie wurde bei CLL-Patienten während des ersten Zyklus häufig ein - teilweise auch klinisch kritisches - Tumorlysesyndrom beobachtet. Deshalb wird auch bei Patienten mit anderen Formen des niedrig malignen NHL, insbesondere wenn Patienten eine hohe Tumormasse aufweisen, empfohlen, beim ersten Zyklus Rituximab am Tag 0 mindestens 24 Stunden vor Beginn der zytostatischen Chemotherapie zu verabreichen, um ein kritisches Tumorlysesyndrom rechtzeitig zu erkennen und nicht durch sofort anschließende Gabe zytostatischer Chemotherapie zu verstärken. Erst wenn beim vorherigen Zyklus keine Zeichen einer klinisch relevanten Tumorlyse erkennbar waren, kann das Intervall zwischen Rituximab und zytostatischer Chemotherapie verkürzt werden und Rituximab an Tag 1 verabreicht werden. Beim R-FCM-Regime wird bei Patienten mit hoher Tumormasse empfohlen, beim ersten Zyklus kein Mitoxantron zu verabreichen, sondern Mitoxantron erst zu verabreichen, wenn beim vorherigen Zyklus keine Zeichen einer klinisch relevanten Tumorlyse erkennbar waren. Bei Anwendung von Fludarabin und den anderen genannten Medikamenten sind die in der jeweiligen Fachinformation enthaltenen Angaben zur Art der Applikation, Verdünnung, Dauer der Infusion, notwendigen Begleitmedikation, Sicherheitsmaßnahmen, Kontraindikationen usw. zu beachten. g) Behandlungsdauer, Anzahl der Zyklen: Bei FCM (Fludarabin, Cyclophosphamid und Mitoxantron) wurden in einer Phase 3-Studie 4 Zyklen in 4-wöchentlichen Abständen als Rezidivtherapie verabreicht (Forstpointner R. et al., 2004). Bei gutem Ansprechen und längerer Remissionsdauer muss bei erneutem Lymphomprogress die Wiederholung der Behandlung mit Nutzen und Risiken der therapeutischen Alternativen abgewogen werden. In der Mehrzahl der Studien wurden nicht mehr als 6, in wenigen Studien auch 8 Zyklen Fludrarabin-haltiger Chemotherapie ununterbrochen in 4- wöchentlichen Abständen gegeben. Aufgrund der starken immunsuppressiven Wirkung von Fludarabin, die durch Kombination mit anderen immunsuppressiv wirksamen Substanzen wie Cyclophosphamid oder Rituximab und durch den krankheitsimmanenten Immundefekt von Patienten mit NHL noch verstärkt wird, kann bei längerer ununterbrochener Gabe ein erhöhtes Risiko für vital bedrohliche Infektionen nicht ausgeschlossen werden. h) Wann sollte die Behandlung abgebrochen werden? Bei Progression der Lymphomerkrankung, fehlendem Ansprechen oder inakzeptabler Toxizität soll Fludarabin abgesetzt werden. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: 11 Bei Kombination von Fludarabin mit anderen immunsuppressiven und/oder hämatotoxischen Substanzen, wie Cyclophosphamid, Mitoxantron oder Rituximab ist mit einer Zunahme der Hämatotoxizität und der Immunsuppression zu rechnen. j) Weitere Besonderheiten: Fludarabin-haltige Therapieschemata sollten ausschließlich von Ärzten / Ärztinnen für Innere Medizin mit dem Schwerpunkt Hämatologie und Onkologie angewandt werden. Aufgrund der starken immunsuppressiven Wirkung von Fludarabin muss die frühzeitige Diagnose und Therapie von Infektionen gewährleistet werden. Zur Pneumocystisprophylaxe bei Therapie mit Purinanaloga wird die Gabe von Cotrimoxazol empfohlen, auch wenn dies in Studien nicht als Standard angegeben bzw. sogar ausdrücklich nicht vorgesehen war. Die Entwicklung einer Hämolyse muss rechtzeitig erkannt und Fludarabin ggf. abgesetzt werden. Bei Lymphomprogress unter Therapie mit Fludarabin wird der Wechsel zu einem anderen Fludarabin- oder Purinanaloga-haltigem Therapieschema nicht empfohlen. So weit angegeben, wurden in den relevanten klinischen Studien zur Erprobung von Fludarabin Patienten, die bereits Fludarabin oder andere Purinanaloga erhalten hatten, ausgeschlossen. Wirksamkeit und Sicherheit eines weiteren Fludarabin- oder Purinanaloga-haltigen Therapieschemas nach Resistenz auf Fludarabin können deshalb nicht bewertet werden. k) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Fludarabin-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: Actavis Nordic A/S und Actavis Group PTC ehf Genzyme Europe B.V. HEXAL AG Neocorp AG TEVA GmbH Nicht verordnungsfähig sind in diesem Zusammenhang die Fludarabin-haltigen Arzneimittel der Firmen Bendalis GmbH, CC-Pharma GmbH, EMRA-MED Arzneimittel GmbH, ESP Pharma Limited, Hospira Deutschland GmbH, Mylan S.A.S, Pharmachemie BV und S.C. Sidan-Pharma S.R.L., da keine entsprechende Erklärung vorliegt. In Bezug auf die Kombination von Fludarabin mit Rituximab hat die Firma Roche Registration Ltd. eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Rituximab-haltigen Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind. Nicht verordnungsfähig sind in diesem Zusammenhang die Rituximab-haltigen Arzneimittel der Firmen AASTON HEALTHCARE GmbH, CC-Pharma GmbH, EMRA-MED Arzneimittel GmbH, EURIM-PHARM Arzneimittel GmbH, kohlpharma GmbH, Pharma Gerke Arzneimittelvertriebs GmbH und Pharma Westen GmbH, da keine entsprechende Erklärung vorliegt. 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Absatz 4 AM-RL: entfällt 12 VII. Etoposid bei Ewing-Tumoren in verschiedenen Kombinationen 1. Heilung bei Erstlinientherapie von Patienten mit Ewing- Tumoren. c) Folgende Wirkstoffe sind zugelassen: Doxorubicinhydrochlorid Cyclophosphamid Vincristinsulfat Ifosfamid Dactinomycin. d) Spezielle Patientengruppe: unvorbehandelt, lokalisiertes Stadium e) Patienten, die nicht behandelt werden sollten: - Patienten mit Kontraindikationen gegen zytostatische Chemotherapie wie schwerwiegende Organinsuffizienzen oder Begleiterkrankungen bzw. massiv reduzierter Allgemeinzustand - Patienten mit metastasiertem Ewing-Tumor - Patienten mit rezidiviertem/refraktärem Ewing-Tumor f) Dosierung: empfohlenes Behandlungsprotokoll Erstlinientherapie bei Patienten mit lokalisiertem Ewing-Tumor: VAC(A) – IE Auf der Basis der Daten der INT-0091-Studie (Grier et al., 2003): Alternierende Therapiezyklen mit VAC(A) Vincristin 2 mg/m² Körperoberfläche (Maximaldosis/Zyklus 2 mg) Tag 1 Doxorubicin (Adriamycin) 75 mg/m² Körperoberfläche Tag 1 Cyclophosphamid 1200 mg/m² Körperoberfläche Tag 1 (+ Mesna) (Actinomycin D 1,25 mg/m² Körperoberfläche Tag 1 ersetzt Doxorubicin nach Erreichen einer kumulativen Doxorubicin-Dosis von 375 mg/m² Körperoberfläche) und IE Ifosfamid 1800 mg/m² Körperoberfläche Tag 1-5 (+ Mesna) Etoposid 100 mg/m² Körperoberfläche i.v. Tag 1-5 abwechselnd alle 3 Wochen. g) Behandlungsdauer: - Primär-/Induktionstherapie: siehe Protokolle INT-0091, EICESS-92, Euro-EWING-99, AEWS0031: Die Gesamtdauer der Therapie erstreckt sich in der Regel über 8-12 Monate. Eine Induktionschemotherapie beinhaltet meist 3-6 Therapiezyklen und wird im Fall lokalisierter Tumoren gefolgt von Operation und/oder Strahlentherapie. 13 h) Wann sollte die Behandlung abgebrochen werden? Die Behandlung sollte abgebrochen werden, wenn vital bedrohliche Nebenwirkungen auftreten, die auch bei optimaler supportiver Therapie, Intervallverlängerung und/oder Dosisreduktion nicht beherrschbar erscheinen. Die Behandlung sollte abgebrochen werden, wenn der Tumor unter Chemotherapie progredient ist oder nach 2 bis 3 Zyklen kein Ansprechen erkennbar ist. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: Bei der Behandlung von Ewing-Tumoren mit Kombinationschemotherapien unter Einschluss u.a. von Etoposid können Zweittumoren auftreten. Das Risiko für sekundäre hämatologische Neoplasien (t-MDS, akute Leukämien, Lymphome) beträgt ca. 1% innerhalb von 5 Jahren. Das Risiko sekundärer Neoplasien unter Einschluss von Etoposid war in den publizierten Studien nicht signifikant gegenüber Therapieprotokollen ohne Etoposid erhöht, sofern konventionelle Dosierungen ohne Hochdosischemotherapie angewendet wurden. j) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Etoposid-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: BERAGENA Arzneimittel GmbH cell Pharm GmbH HEXAL AG HIKMA Farmaceutica (Portugal), S.A. medac Gesellschaft für klinische Spezialpräparate mbH Neocorp AG Pharmachemie BV TEVA GmbH. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Absatz 4 AM-RL: entfällt“ 14 VIII. Doxorubicin beim Merkelzellkarzinom 1. Hinweise zur Anwendung von Doxorubicin beim Merkelzellkarzinom gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): palliative Therapie des disseminierten oder lokoregionär fortgeschrittenen/ inoperablen Merkelzellkarzinoms b) Behandlungsziel: Erreichen einer Remission, Verlängerung der Remissionsdauer im Falle eines Ansprechens auf die Therapie c) Folgende Wirkstoffe sind zugelassen: keine d) Spezielle Patientengruppe: Für Chemotherapie bei lokal fortgeschrittenem Tumor kommen nur Patienten in Betracht, bei denen die lokalen Behandlungsmöglichkeiten mit Operation und/oder Radiatio ausgeschöpft oder nicht möglich sind. Chemotherapie kann unter dieser Voraussetzung bei lokalisierter Erkrankung oder bei metastasierter Erkrankung sowohl in der Erstlinie als auch in einer späteren Therapielinie eingesetzt werden. Keine Altersbegrenzung. e) Patienten, die nicht behandelt werden sollten: Patienten mit Kontraindikationen für anthrazyklinhaltige Chemotherapie, insbesondere echokardiographisch mittel- oder hochgradig eingeschränkte Myokardfunktion, sollten kein Doxorubicin erhalten (siehe Fachinformation zu Doxorubicin). Der Stellenwert einer adjuvanten Chemotherapie ist nicht belegt, weder nach Operation noch nach Bestrahlung, sodass diese Patienten nicht mit Doxorubicin behandelt werden sollten. Auch eine Kombination von Doxorubicin mit Radiatio im Sinne einer Radiochemotherapie wird außerhalb klinischer Studien nicht empfohlen, da Nutzen und Risiken nicht bewertet werden können. f) Dosierung: Empfohlen wird die Kombination von Doxorubicin mit Cyclophosphamid und Vincristin (CAV-Schema): Cyclophosphamid 1000 mg/m2 Körperoberfläche Tag 1 Doxorubicin (Adriamycin) 45 - 50 mg/m2 Körperoberfläche Tag 1 Vincristin 1,4 mg/m2 Körperoberfläche (maximale Dosis 2 mg) Tag 1 Wiederholung Tag 22. g) Behandlungsdauer: Zur Anzahl der Therapiezyklen bzw. Dauer der Behandlung bei Patienten mit Merkelzellkarzinom existieren keine publizierten Daten. In Analogie zur Therapie des kleinzelligen Bronchialkarzinoms wird empfohlen bei Patienten, die auf die Behandlung ansprechen, 6 Zyklen CAV zu verabreichen. h) Wann sollte die Behandlung abgebrochen werden? - Bei fehlendem Ansprechen nach 2 Kursen CAV oder bei Progression des Tumors unter Chemotherapie. - Bei trotz optimaler supportiver Therapie, Verlängerung der Applikationsintervalle und/oder Dosisreduktion nicht ausreichend beherrschbaren, klinisch kritischen Nebenwirkungen. 15 - Bei Erreichen einer Doxorubicin-Gesamtdosis von 550 mg/m2 Körperoberfläche und/oder echokardiographisch dokumentierter, klinisch relevanter Verschlechterung der linksventrikulären Funktion (siehe oben). i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: Nein. j) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Doxorubicin-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: Actavis Group PTC ehf cell Pharm GmbH EBEWE Pharma Ges.m.b.H. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4 AM-RL: entfällt 16 IX. Verapamil zur Prophylaxe des Clusterkopfschmerzes 1. Hinweise zur Anwendung von Verapamil zur Prophylaxe des Clusterkopfschmerzes gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Prophylaxe des episodischen und chronischen Clusterkopfschmerzes von Erwachsenen ab 18 Jahren b) Behandlungsziel: klinisch relevante Reduzierung (d. h. wenigstens 50%) der Frequenz von Attacken des Clusterkopfschmerzes c) Folgende Wirkstoffe sind zugelassen: Lithiumacetat und Lithiumcarbonat d) Spezielle Patientengruppe: Erwachsene mit Clusterkopfschmerz unabhängig von der Verlaufsform. Frauen im gebärfähigen Alter müssen durch den behandelnden Arzt über das erhöhte Risiko von Missbildungen sowie darüber aufgeklärt werden, dass während der Behandlung mit Verapamil eine effektive Methode der Kontrazeption durchgeführt werden sollte. Es wird verwiesen auf Punkt 4.6 Schwangerschaft und Stillzeit der Fachinformation: „Wegen der Gefahr schwerwiegender Nebenwirkungen beim Säugling sollte Verapamil während der Stillzeit nur dann verwendet werden, wenn dies für das Wohlergehen der Mutter unbedingt notwendig ist“. e) Patienten, die nicht behandelt werden sollten: Gegenanzeigen und Wechselwirkungen entsprechen denen der Fachinformation. Dazu gehören Reizleitungsstörungen, Vorhofflimmern/- flattern, manifeste Herzinsuffizienz und Myokardinfarkt sowie Patienten, die mit Betablockern behandelt werden, Patienten mit chronisch paroxysmaler Hemikranie und Patienten mit Medikamentenfehlgebrauch. f) Dosierung: Es wird eine orale Monotherapie mit einer anfänglichen Tagesdosis von 120 mg empfohlen, die bis 360 mg gesteigert werden kann. Tagesdosen über 480 mg sind nicht ausreichend untersucht. Die Dosis von Verapamil, die für eine vollständige Unterdrückung der Attacken des Clusterkopfschmerzes notwendig ist, ist interindividuell sehr unterschiedlich und kann auch intraindividuell schwanken. g) Behandlungsdauer: Die therapeutische Wirksamkeit kann frühestens nach einer Behandlungsdauer von einer Woche beurteilt werden. In der Regel wird die Dauer der Therapie beim episodischen Clusterkopfschmerz ca. 6 Wochen betragen, beim chronischen Clusterkopfschmerz ist eine Dauertherapie erforderlich. Hierzu ist ein Schmerztagebuch durch den Patienten zu führen. h) Wann sollte die Behandlung abgebrochen werden? Neben den in der Fachinformation aufgeführten Gründen sollte die Behandlung abgebrochen werden, wenn das Therapieziel einer 50 %igen Reduktion der Attackenfrequenz nicht erreicht wird. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: 17 Es wird darauf hingewiesen, dass sich aufgrund der zumeist eingesetzten höheren Dosierungen häufiger als bei anderen Patientengruppen EKG- Veränderungen und Herzrhythmusstörungen zeigen, weshalb regelmäßige EKG-Kontrollen während der Prophylaxe mit Verapamil indiziert sind. In den bewerteten Studien wurde über keine Nebenwirkungen bzw. Wechselwirkungen, die über die Fachinformation hinausgehen, berichtet. j) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Verapamil-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: 1 A Pharma GmbH, Abbot GmbH & Co. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4 AM-RL: Während der Prophylaxe des Clusterkopfschmerzes mit Verapamil sind regelmäßige EKG-Kontrollen durchzuführen und zu dokumentieren. 18 X. Clostridium botulinum Toxin Typ A (OnabotulinumtoxinA, AbobotulinumtoxinA) bei Spasmodischer Dysphonie (Laryngealer Dystonie) 1. Hinweise zur Anwendung von Clostridium botulinum Toxin Typ A bei Spasmodischer Dysphonie (Laryngealer Dystonie) gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Spasmodische Dysphonie vom Adduktortyp b) Behandlungsziel: Verbesserung der Kommunikationsfähigkeit durch lokale Schwächung der dystonen Muskulatur des Larynx Linderung der klinischen Beschwerdesymptomatik (Dysphonie, Dyspnoe) c) Folgende Wirkstoffe sind zugelassen: keine d) Spezielle Patientengruppe: Geeignet für die Behandlung mit Clostridium botulinum Toxin Typ A sind Patienten mit spasmodischen Dysphonien, bei denen keine Unverträglichkeit gegen Botulinumtoxin A vorliegt. Die Wirksamkeit ist für die spasmodische Dysphonie vom Adduktortyp belegt. Vor einer Therapie ist eine interdisziplinäre Abklärung der Diagnose unter Beteiligung von Hals-Nasen-Ohren-Ärzten, Neurologen, Psychiatern und Internisten erforderlich. Chirurgische Vorbehandlungen stellen hierbei kein Ausschlusskriterium dar. e) Patienten, die nicht behandelt werden sollten: Als Einschränkungen wären zentrale neurodegenerative Erkrankungen sowie neuromuskuläre Grunderkrankungen zu nennen, die als Ursache der Dysphonie oder zusätzlich zur Dysphonie bestehen sowie Dysphagien mit Aspirationsneigung, weil sich diese durch die Toxinbehandlung verschlimmern könnten. Wenn Infektionen am Injektionsort vorliegen oder die Patienten an einem Engwinkelglaukom leiden, sollten keine Injektionen erfolgen. Eine gleichzeitige antibiotische Therapie mit Aminoglykosiden und auch die Einnahme anticholinerger Medikamente (Muskelrelaxantien, Atropin) ist aufgrund möglicher Wirkverstärkungen des Clostridium botulinum Toxin Typ A zu vermeiden. Des Weiteren sollten bei den Patienten keine verstärkten Blutungsneigungen bestehen, z. B. auch aufgrund der Einnahme gerinnungshemmender Medikamente (Acetylsalicylsäure, Phenprocoumon, Valproinsäure). Die Patienten müssen aufgrund ihres Allgemeinzustandes entweder zur Kooperation bei der Behandlung in Oberflächenanästhesie fähig oder für eine Therapie in Narkose geeignet sein. Schwangere und Stillende sollten aufgrund der unzureichenden Kenntnislage von einer Therapie ausgeschlossen werden. Dies begründet sich aus der möglichen Toxinwirkung auf das Ungeborene bzw. Neugeborene sowie auf muskuläre Organe der Behandelten, die während der Geburt aktiv sind. Hierzu gehört auch der Larynx bei der Erhöhung des intraabdominellen Druckes. Aufgrund der Altersstrukturen dieser beiden Gruppen (Schwangere bzw. 19 Stillende und Patientinnen mit spasmodischer Dysphonie) sind Überschneidungen selten. f) Dosierung: 2,5 ME OnabotulinumtoxinA (Botox®) pro injizierten Kehlkopfmuskel bis zu einer Höchstdosis von 10 ME mit Wiederholungsbehandlungen alle 3 bis 6 Monate als Monotherapie [Dressler 2010]. Die wirksame Dosis für AbobotulinumtoxinA (Dysport®) wird mit 3 – 3,75 ME angegeben [Whurr 1993]. Die umfangreichste Erfahrung liegt mit den Techniken zur endoskopischen Injektion vor, eine transcutane Injektion ist ebenfalls wirksam. g) Behandlungsdauer: Nicht begrenzt. Auch nach langjährigen, fortlaufenden Behandlungen wurden weder Wirkungsverluste beschrieben, die zu stetig steigenden Toxindosierungen führten, noch Zunahmen oder Neuauftreten von Nebenwirkungen, die durch die Dauerbehandlung entstanden und einen Behandlungsabbruch erforderten [Novakovic 2011, Blitzer 2010, Boutsen 2002]. h) Wann sollte die Behandlung abgebrochen werden? Bei fehlendem Erfolg, bei Nebenwirkungen wie Dysphagien mit Aspirationsneigung. In beiden Fällen kann zunächst mit einer reduzierten angepassten Dosis reagiert werden. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: Eine vorübergehende initiale Heiserkeit kommt vor, ist aber Teil des beabsichtigten Wirkungsprofils. j) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre OnabotulinumtoxinA- und AbobotulinumtoxinA-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: Allergan Pharmaceuticals Ireland, EMRAmed Arzneimittel GmbH, EurimPharm Arzneimittel GmbH, IPSEN PHARMA GmbH und VERON PHARMA Vertriebs GmbH. Nicht verordnungsfähig sind in diesem Zusammenhang die OnabotulinumtoxinA- und AbobotulinumtoxinA-haltigen Arzneimittel der Firmen AASTON HEALTHCARE GmbH, AxiCorp Pharma GmbH, A.C.A. Müller ADAG Pharma AG, BERAGENA Arzneimittel GmbH, CC Pharma GmbH, HAEMATO PHARMA AG, Kohlpharma GmbH, Medicopharm AG und Pharma Westen GmbH, da keine entsprechende Erklärung vorliegt. 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4 AM-RL: entfällt 20 XI. Irinotecan bei kleinzelligem Bronchialkarzinom (SCLC), extensive disease 1. Hinweise zur Anwendung von Irinotecan bei kleinzelligem Bronchialkarzinom (SCLC), extensive disease gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Irinotecan in Kombination mit einem Platinpräparat bei Patienten mit kleinzelligem Bronchialkarzinom (SCLC), extensive disease b) Behandlungsziel: palliative Therapie zur Verlängerung der Überlebenszeit c) Folgende Wirkstoffe sind zugelassen: Epirubicin, Doxorubicin, Etoposid, Carboplatin, Cisplatin, Vincristin, Ifosfamid, Lomustin, Cyclophosphamid, Topotecan d) Spezielle Patientengruppe: Die Kombination von Irinotecan mit einem Platinpräparat kann eingesetzt werden bei Patienten, die in der Erstlinientherapie ein Platinpräparat und Etoposid erhalten haben und bei denen so schwerwiegende, etoposidbedingte Nebenwirkungen beobachtet wurden, dass die weitere Gabe von Etoposid mit inakzeptablen Risiken verbunden wäre. e) Patienten, die nicht behandelt werden sollten: Patienten, die in der Fachinformation von Irinotecan genannte Kontraindikationen aufweisen. f) Dosierung: Irinotecan sollte bei dreiwöchigem Zyklusintervall in einer Dosierung von 65 mg/m2 Körperoberfläche an Tag 1 und 8 in der Regel in Kombination mit Cisplatin 60 - 80 mg/m2 Körperoberfläche an Tag 1 appliziert werden. g) Behandlungsdauer, Anzahl der Zyklen: Es sollten maximal 6 Behandlungszyklen verabreicht werden. h) Wann sollte die Behandlung abgebrochen werden? Die Behandlung sollte bei Tumorprogress oder vital bedrohlicher Toxizität abgebrochen werden. Die am häufigsten beschriebene, vital bedrohliche Toxizität von Irinote-can ist Diarrhoe mit Exsikkose (siehe Fachinformation). i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: Die Kombination von Strahlentherapie mit Irinotecan wurde in klinischen Studien bei Patienten mit Rektum- und Bronchialkarzinom erprobt. B. mit der „Medical Research Council“ Skala), zusätzlich bei Dermatomyositis Verbesserung der Hautveränderungen (Fotodokumentation) c) Folgende Wirkstoffe sind zugelassen: - Polymyositis: Prednisolon und Prednison, Azathioprin - Dermatomyositis: Methylprednisolon, Prednisolon und Prednison, Dexamethason, Azathioprin, Triamcinolon d) Spezielle Patientengruppe: Erwachsene Patienten mit gesicherter Diagnose einer Polymyositis oder Dermatomyositis (Leitlinien der DGN 2008), die unter der zugelassenen Therapie eine Therapieresistenz zeigen. Therapieresistenz liegt in der Initialphase vor, wenn Kortikoide in Monotherapie z. B. bei einem foudroyanten Verlauf, nicht wirksam sind, d. h. in Aktivitäten des alltäglichen Lebens, in der Langzeittherapie in der Kombination mit Azathioprin eine Absenkung der Kortikoiddosis unter die Cushing-Schwelle nicht gelingt oder wenn 6 Monate nach Eindosierung von Azathioprin zu einer laufenden Steroidtherapie eine klinische Besserung ausbleibt. Bei erwachsenen Patienten mit lebensbedrohlicher Dermatomyositis, z. B. Atemlähmung oder schwerwiegende Schluckstörung, kann IVIG als Kombinationspartner erwogen werden. e) Patienten, die nicht behandelt werden sollten: Gegenanzeigen entsprechen denen der Fachinformation. f) Dosierung: IVIG 1 g/kg Körpergewicht zweimal pro Monat (0,4 g/kg Körpergewicht pro Tag für 5 Tage) g) Behandlungsdauer: - bei Respondern mindestens 6 Monate (siehe DALAKAS et al. 1993, CHERIN et al. 2002), nach Wirkung auch länger - bei Nonrespondern nach 3 Monaten Abbruch, Kriterien für Therapieresponse bei Polymyositis und Dermatomyositis: klinisch relevante Besserung der 23 Muskelkraft, Abnahme der Creatin-Kinase-Werte im Verlauf, bei Dermatomyositis zusätzlich dazu Verbesserung der Hautveränderungen h) Wann sollte die Behandlung abgebrochen werden? - siehe Fachinformation - bei Nonrespondern nach 3 Zyklen (3 Monaten). i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: keine j) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre IVIG-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: Baxter Deutschland GmbH, Biotest Pharma GmbH, CSL Behring GmbH, EMRAmed Arzneimittel GmbH, Grifols Deutschland GmbH; Kedrion S.p.A., MPA Pharma GmbH und Octapharm GmbH. Nicht verordnungsfähig sind in diesem Zusammenhang die IVIG-haltigen Arzneimittel der Firmen A.S. Unicare Pharma-Vertriebs GmbH, CC-Pharma GmbH, EURIM-Pharm Arzneimittel GmbH und Pharma Gerke Arzneimittelvertriebs GmbH, da keine entsprechende Erklärung vorliegt. 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4 AM-RL: entfällt. 24 XIII. 5-Fluorouracil in Kombination mit Mitomycin und Bestrahlung bei Analkarzinom 1. Hinweise zur Anwendung von 5-Fluorouracil in Kombination mit Mitomycin und Bestrahlung bei Analkarzinom gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Die Behandlung mit 5-Fluorouracil (5-FU) und Mitomycin parallel zur Strahlen- therapie ist angezeigt für nicht vorbehandelte Patienten mit Analkarzinom, bei denen die potenziell kurative operative Behandlung zum Verlust des natürlichen Darmausgangs führen würde. Die Behandlung mit 5-Fluorouracil (5-FU) und Mitomycin parallel zur Strahlen- therapie ist angezeigt für Patienten mit Rezidiv eines Analkarzinoms nach operativer Behandlung. b) Behandlungsziel: Vermeidung der abdominoperinealen Rektumresektion und der dadurch notwendigen Anlage eines Kolostomas; damit Verbesserung der Lebensqualität durch Erhalt des natürlichen Darmausgangs. Im Vergleich zur alleinigen Radiatio: Verbesserung der lokalen Kontrollrate und Verlängerung des krankheitsspezifischen Überlebens. c) Folgende Wirkstoffe sind zugelassen: Keine. d) Spezielle Patientengruppe: Erstlinien-Therapie von Patienten mit Analkanalkarzinom, bei denen die Indikation zur Radiotherapie gestellt wurde. Für Patienten mit einem Allgemeinzustand ECOG > 1 oder der Histologie eines Adenokarzinoms fehlen Daten klinischer Studien zur Bewertung der Radiochemotherapie. e) Patienten, die nicht behandelt werden sollten: Patienten mit Kontraindikationen, wie in den Fachinformationen der beiden Medikamente angegeben. f) Dosierung: Nach dem RTOG/ECOG-Protokoll (Flam et al., 1996): 5-Fluorouracil 1000 mg/m² als Infusion über 24 Stunden Tage 1 bis 4 und Tage 29 bis 32, maximale Tagesdosis absolut 2 000 mg. Mitomycin 10 mg/m² Tage 1 und 29 als Bolus i.v., maximale Dosis absolut 20 mg. Die Chemotherapie wird an den Tagen 1 bis 4 und an den Tagen 29 bis 32 (1. und 5. Woche) der Strahlentherapie verabreicht. Gesamtdosis der Bestrahlung 45 Gy, 1,8 Gy täglich über 5 Wochen, Boost 5,4-9 Gy abhängig von Tumorgröße, Nodalstatus und Remission (Details siehe Original- publikation von Flam, 1996). g) Behandlungsdauer, Anzahl der Zyklen: Siehe oben. h) Wann sollte die Behandlung abgebrochen werden? Bei einzelnen Patienten kann es zu einer schweren chemotherapie-induzierten Mukositis kommen. Wenn diese sich in seltenen Fällen schon während der ersten 4- 25 tägigen Chemotherapiephase mit Schmerzen in der Mundschleimhaut manifestiert, kann erwogen werden, bei Fortführung der Bestrahlung auf die Gabe des zweiten Zyklus Chemotherapie zu verzichten oder die 5-FU-Dosis zu reduzieren. Bei lebensbedrohlichen Nebenwirkungen (insbesondere schweren Infektionen, pulmonale Toxizität, Nephrotoxizität etc.) sollte auf den 2. Zyklus Chemotherapie verzichtet werden. Wenn möglich sollte die Strahlenbehandlung fortgeführt werden. Abbruch der Chemotherapie mit 5-FU bei pektanginösen Beschwerden oder anderen ischämisch-vaskulären Ereignissen (z. B. Apoplex). Mitomycin-Dosisreduktion im Zyklus 2 nach Grad-3/4-Hämatotoxizität nach Zyklus 1, bei höhergradiger Hämatotoxizität ggf. auch Unterbrechung der Radiochemotherapie für 1 Woche. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: Besonders zu beachten sind synergistische Toxizitäten durch parallele Anwendung von Strahlentherapie und zytostatischer Chemotherapie, insbesondere Schleimhaut- Toxizität im Bestrahlungsfeld. Bei Patienten mit Analkarzinom auf der Basis einer Immunsuppression oder einer Immundefizienz muss die Indikationsstellung zur Chemotherapie besonders kritisch gestellt werden und die Überwachung unter laufender Therapie besonders sorgfältig sein, z. B. frühzeitige Diagnose von Infektionen. j) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre 5-Fluorouracil- und/oder Mitomycin-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: APOGEPHA Arzneimittel GmbH, axios Pharma GmbH, BHARDWAJ PHARMA GmbH, biosyn Arzneimittel GmbH, cell pharm GmbH, EBEWE Pharma Ges.m.b.H. Nicht verordnungsfähig sind in diesem Zusammenhang die 5-Fluorouracil- und Mitomycin -haltigen Arzneimittel der Firmen Bendalis GmbH, BERAGENA Arzneimittel GmbH, CC-Pharma GmbH, Hospira Deutschland GmbH, KOHNE PHARMA GmbH, Onkopharm Arzneimittel GmbH, OTP Oncotrade Parenteralia GmbH, PGD Profusio Leipzig Gesundheits GmbH und Speciality European Pharma Limited, da keine entsprechende Erklärung vorliegt. 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4 AM-RL: entfällt. 26 XIV. Hydroxycarbamid bei chronischer myelomonozytärer Leukämie 1. Hinweise zur Anwendung von Hydroxycarbamid bei chronischer myelomonozytärer Leukämie gemäß § 30 Absatz 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Patienten/innen mit einer chronischen myelomonozytären Leukämie (CMML), defi- niert nach der FAB-Klassifikation mit einer Monozytose > 1.000/μl im Blut und einem Blastenanteil im Knochenmark < 30 %, bei denen eine Indikation für eine zytostatische Therapie besteht (siehe „Spezielle Patientengruppe“) oder bei Patienten mit CMML nach Übergang in eine akute myeloische Leukämie (AML, Blastenanteil im Knochenmark ≥ 30 %), die eine Kontraindikation für eine aggressive Induktionschemotherapie mit einem konventionellen AML-Protokoll aufweisen. b) Behandlungsziel: Palliative Therapie zur Überlebenszeitverlängerung c) Folgende Wirkstoffe sind zugelassen: Für eine Untergruppe der CMML-Patienten/innen ist 5-Azacitidine zugelassen: Patienten, die nicht für eine Behandlung mit allogener Stammzelltransplantation geeignet sind und eine CMML mit 10 – 29 % Knochenmarkblasten ohne myeloproliferative Störung aufweisen. d) Spezielle Patientengruppe: CMML-Patienten/innen, bei denen eine Indikation zur zytostatischen Chemotherapie besteht. Eine Indikation zur zytostatischen Chemotherapie besteht in der Regel, wenn zwei der folgenden Merkmale vorliegen: − Leukozyten > 16.000/μl, Hämoglobin < 10 g/dl, Thrombozyten < 100.000/μl, Blasten im Knochenmark > 5 %, Splenomegalie > 5 cm unterhalb Rippenbogen und / oder wenn eines der folgenden Merkmale vorliegt: − zytologisch oder histologisch nachgewiesene Beteiligung anderer Organe als Milz, Leber und Lymphknoten, histologisch gesicherte Hautbeteiligung, zytologisch gesicherter Befall bei Pleura- / Perikarderguss oder Aszites. Diese Merkmale sichern, dass keine Niedrigrisikopatienten therapiert werden. Nach heutigem Kenntnisstand sind zudem erhöhter Laktatdehydrogenase-Wert und ungünstiger Karyotyp als weitere Risikomerkmale zu nennen. e) Patienten, die nicht behandelt werden sollten: Patienten mit Leukozyten < 5.000/μl, sofern keine zytologisch oder histologisch nachgewiesene therapiebedürftige Organbeteiligung vorliegt (siehe „Spezielle Patientengruppe“). f) Dosierung: Initiale Dosis: 2 x 500 mg Hydroxycarbamid per os täglich. Bei viszeraler Beteiligung, drohendem oder bereits erfolgten AML-Übergang: initiale Dosis 2 x 1.000 mg Hydroxycarbamid per os täglich. Die weitere Dosierung soll dem Leukozytenverlauf angepasst werden. Es sollen Leukozytenwerte zwischen 5.000/μl und 10.000/μl angestrebt werden. In den 27 Dosierungsempfehlungen der Phase 3-Studie von Wattel et al. (1996) wurde als maximale Tagesdosis 2 x 2 g angegeben. Bei ausgeprägter Granulo- und/oder Thrombozytopenie sind engmaschige Blutbild- kontrollen erforderlich und rechtzeitig eine Dosisreduktion von Hydroxycarbamid bzw. supportive Maßnahmen wie Antibiotikaprophylaxe und/oder Thrombozytentransfusionen in Erwägung zu ziehen. g) Behandlungsdauer: Es handelt sich um eine orale Dauertherapie, die so lange fortgeführt wird, wie die CMML ausreichend kontrolliert werden kann. h) Wann sollte die Behandlung abgebrochen werden? Die Hydroxycarbamidtherapie soll abgebrochen werden, wenn auch bei der maximal tolerablen Dosis eine ausreichende Kontrolle der Leukozytose oder der Organinfiltration nicht (mehr) erreicht werden kann. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: Insbesondere bei ausgeprägter Leukozytose muss mit der Entwicklung eines Tumorlysesyndroms gerechnet werden. Deshalb sind entsprechende Vorsichtsmaßnahmen zu ergreifen, eine ausreichende Diurese ist zu gewährleisten und ggf. die Gabe von Allopurinol in Betracht zu ziehen. Häufige Nebenwirkungen sind Granulozytopenie, Anämie, Thrombozytopenie und Hautreaktionen. Die Fachinformation ist unbedingt zu beachten. j) Weitere Besonderheiten Die Behandlung soll von einem Facharzt/ einer Fachärztin für Innere Medizin, Hämatologie und Onkologie durchgeführt werden. k) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Hydroxycarbamid-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: 1 A Pharma GmbH, axicorp Pharma GmbH, EMRAmed Arzneimittel GmbH, EurimPharm Arzneimittel GmbH, Hexal AG und medac Gesellschaft für klinische Spezialpräparate mbH. Nicht verordnungsfähig sind in diesem Zusammenhang die Hydroxycarbamid- haltigen Arzneimittel der Firmen A.C.A. Müller ADAG Pharma AG, Addmedica, ADL Pharma GmbH, BERAGENA Arzneimittel GmbH, Bristol-Myers Squibb GmbH, CC- Pharma GmbH, kohlpharma GmbH, Medicopharm AG und Pharma Westen GmbH, da keine entsprechende Erklärung vorliegt. 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4 AM-RL: entfällt 28 XV. Imiquimod zur Behandlung analer Dysplasien als Präkanzerosen bei HIV 1. Hinweise zur Anwendung von Imiquimod zur Behandlung analer Dysplasien als Präkanzerosen bei HIV gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Imiquimod zur Behandlung hochgradiger analer intraepithelialer Dysplasien (AIN) als Präkanzerosen bei HIV-infizierten Personen unter antiretroviraler Therapie. b) Behandlungsziel: Rückbildung oder „Downgrading“ der AIN c) Folgende Wirkstoffe sind zugelassen: Es sind im Geltungsbereich des AMG keine Arzneimittel zur Behandlung analer Dysplasien als Präkanzerosen bei HIV zugelassen. d) Spezielle Patientengruppe: Nachweis einer HIV-Infektion bei den Patienten, unabhängig vom Virusload und der aktuellen CD4-Zellzahl. Keine Einschränkungen hinsichtlich der beteiligten HPV-8 Typen oder opportunistischer Infektionen. e) Patienten, die nicht behandelt werden sollten: Patienten mit bekannter Überempfindlichkeit gegen lmiquimod. f) Dosierung: ½ Sachet Imiquimod-Creme 5% topisch 3x pro Woche; bei starken lokalen Reizungen Aussetzen für mehrere Tage bis zum Abklingen der Reizung. g) Behandlungsdauer: Behandlungsdauer mind. 16 Wochen, bei Nicht-Ansprechen Fortführung für weitere 16 Wochen. h) Wann sollte die Behandlung abgebrochen werden? Die Behandlung ist nach 32 Wochen Therapie zu beenden, wenn kein Therapieerfolg beobachtet wird. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: Sind nicht bekannt. j) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Imiquimod-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: axicorp Pharma GmbH, EMRAmed Arzneimittel GmbH, EurimPharm Arzneimittel GmbH, MEDA Pharma GmbH & Co. KG und Pharma Gerke GmbH. Nicht verordnungsfähig sind in diesem Zusammenhang die Imiquimod-haltigen Arzneimittel der Firmen BERAGENA Arzneimittel GmbH, CC-Pharma GmbH, kohlpharma GmbH, Laboratoires 3M, Medicopharm AG und Pharma Westen GmbH, da keine entsprechende Erklärung vorliegt. 29 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Absatz 4 AM-RL: entfällt 30 XVI. Mycophenolat Mofetil bei Myasthenia gravis 1. Hinweise zur Anwendung von Mycophenolat Mofetil bei Myasthenia gravis gemäß § 30 Abs. 2 AM-RL: a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Langzeittherapie bei generalisierter Myasthenia gravis bei Therapieresistenz unter Behandlung mit den zugelassenen Substanzen oder bei Azathioprin- Unverträglichkeit. b) Behandlungsziel: Aufrechterhaltung oder Besserung der Remission c) Welche Wirkstoffe sind zugelassen: Pyridostigminbromid, Neostigminmetilsulfat, Distigminbromid Prednisolon, Prednison Azathioprin d) Spezielle Patientengruppe: Patienten mit generalisierter Myasthenia gravis und der klinischen Notwendigkeit zu einer lang anhaltenden Immunsuppression, bei denen sich Azathioprin als unverträglich erwiesen hat oder bei denen sich Azathioprin bei einer ausreichend dosierten Therapie als nicht ausreichend wirksam erwiesen hat (Therapieresistenz) oder eine Absenkung der begleitenden Kortikoid-Dosis unter die Cushingschwelle nicht erreichbar war. e) Patienten, die nicht behandelt werden sollen: Patienten, auf die die unter d) genannten Kriterien nicht zutreffen. Es gibt keine Hinweise, dass Mycophenolat Mofetil in der myasthenen Krise wirksam ist. In der frühen Phase der Immunsuppression (induction remission) zeigt Mycophenolat Mofetil in Kombination mit Kortikoiden keinen Zusatznutzen im Vergleich zur Monotherapie mit Kortikoiden. Für eine Wirksamkeit von Mycophenolat Mofetil als Monotherapie zur primären Immunsuppression bei Myasthenia gravis liegt bislang keine ausreichende Evidenz vor. f) Dosierung: In der Literatur gibt es keine einheitlichen Empfehlungen zur Dosierung von Mycophenolat bei Myasthenia gravis. Die Therapie muss fallindividuell nach klinischem Verlauf angepasst erfolgen. Zumeist wurde Mycophenolat Mofetil bei generalisierter Myasthenia gravis in einer Tagesdosis von 0,5-3 g eingesetzt. g) Behandlungsdauer: Die Therapiedauer richtet sich nach dem individuellen Krankheitsverlauf und der Verträglichkeit von Mycophenolat Mofetil. Ein Wirkungseintritt ist nach einer Behandlungszeit von 12 Monaten zu erwarten. Im Krankheitsverlauf bei Myasthenia gravis muss regelmäßig klinisch überprüft werden, inwieweit die Notwendigkeit zu einer Immunsuppression fortbesteht bzw. die angewandte Dosierung weiterhin erforderlich ist oder reduziert werden kann. h) Wann sollte die Behandlung abgebrochen werden? Die Therapie muss gemäß den Vorgaben der Fachinformation überwacht werden. Bei nicht tolerierbaren Nebenwirkungen (siehe Fachinformation) ist gegebenenfalls der sofortige Therapieabbruch erforderlich. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: 31 Auch wenn es keinen Anhalt dafür gibt, dass es beim Einsatz von Mycophenolat Mofetil bei Myasthenia gravis zu Nebenwirkungen/Wechselwirkungen kommt, die über das in der Fachinformation beschriebene Maß hinausgehen, wird empfohlen, Mycophenolat Mofetil nicht zusammen mit Azathioprin zu verabreichen, da die gleichzeitige Behandlung mit diesen Präparaten nicht untersucht worden ist (siehe Fachinformation Mycophenolat Mofetil) und pharmakologisch nicht sinnvoll erscheint. j) Weitere Besonderheiten: Die Therapie einer Myasthenia gravis mit Mycophenolat Mofetil darf ausschließlich durch eine/einen im Krankheitsbild erfahrene(n) Fachärztin/Facharzt für Nervenheilkunde oder Fachärztin/Facharzt für Neurologie oder Nervenärztin/Nervenarzt erfolgen. k) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Mycophenolat Mofetil- haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: 1A Pharma GmbH, EURIM-PHARM Arzneimittel GmbH, HEXAL AG, Roche Registration Ltd. Nicht verordnungsfähig sind in diesem Zusammenhang die Mycophenolat Mofetil- haltigen Arzneimittel der Firmen ACA Müller ADAG Pharma AG, BERAGENA Arzneimittel GmbH, CC-Pharma GmbH, EMRA-MED Arzneimittel GmbH, kohlpharma GmbH, MPA Pharma GmbH, MTK-PHARMA-Vertriebs-GmbH, Pharma Gerke Arzneimittelvertriebs GmbH, Pharma Westen GmbH, Sandoz Pharmaceuticals GmbH, Teva Pharma B. V., Winthrop Arzneimittel GmbH, da keine entsprechende Erklärung vorliegt. 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4 AM-RL: entfällt. 32 XVII. unbesetzt 33 XVIII. Amikacin bei Tuberkulose 1. Hinweise zur Anwendung von Amikacin bei Tuberkulose gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Es kann als erwiesen gelten, dass Amikacin antituberkulöse Wirkung in vitro und in vivo hat. Wie für alle anderen Tuberkulosemittel gilt, dass diese Wirkung nur im Rahmen von Kombinationstherapien klinisch erfolgreich sein kann. b) Behandlungsziel: Therapie/Heilung von Patienten mit einer Tuberkuloseerkrankung, die gegen eine zugelassene antituberkulöse Therapie resistent ist. Wie für alle anderen Tuberkulosemittel gilt, dass diese Wirkung nur im Rahmen von Kombinationstherapien klinisch erfolgreich sein kann. c) Folgende Wirkstoffe sind zugelassen: 4-Aminosalicylsäure (PAS), Betamethason, Dexamethason, Ethambutol, Ethambutol + Isoniazid, Isoniazid, Isoniazid + Pyrazinamid, Isoniazid + Vitamin B6, Prednison, Protionamid, Pyrazinamid, Rifabutin, Rifampicin, Rifampicin + Isoniazid, Rifampicin + Isoniazid + Pyrazinamid, Streptomycin, Terizidon d) Spezielle Patientengruppe: Es dürfen nur Patienten behandelt werden, bei denen eine Tuberkuloseerkrankung vorliegt, und bei denen - aufgrund des Resistenzmusters der Tuberkelbakterien oder - aufgrund der Anamnese, die auf eine Resistenz (Index oder wahrscheinlich ein Index-Patient mit Resistenz-Tuberkulose) schließen lässt oder - aufgrund von Nebenwirkungen keine andere Therapie zur Verfügung steht. Der Allgemeinzustand des Patienten muss für eine solche Therapie ausreichend sein. e) Patienten, die nicht behandelt werden sollen: Es dürfen nur Patienten mit oben genannter Indikation behandelt werden. f) Dosierung (im Rahmen der Kombinationstherapie): Amikacin wird parenteral appliziert. Die tägliche Dosis liegt bei 10 – 15 mg/kg Körpergewicht (KG). Die Anwendung ist streng an das Vorliegen der Mehrfachresistenz (MDR) / Extensiven Resistenz (XDR) – Situation und an die Materialentnahme zur Resistenzbestimmung/ das Vorliegen einer Resistenzprüfung der Tb-Erreger gebunden. g) Behandlungsdauer: 34 Zielkriterium ist die Heilung, dementsprechend bis keine Erreger mehr nachgewiesen werden können. h) Wann sollte die Behandlung abgebrochen werden? Ein Abbruch der Behandlung wegen lebensbedrohlicher/intolerabler Nebenwirkungen sollte gemeinsam mit dem Patienten abgewogen werden unter Berücksichtigung des Vorliegens einer lebensbedrohlichen Erkrankung und der fehlenden Therapiemöglichkeiten. i) Nebenwirkungen/Wechselwirkungen, wenn diese über die zugelassene Fachinformation hinausgehen oder dort nicht erwähnt sind: Die Nebenwirkungsrisiken (Nephro- und Ototoxizität und Gleichgewichtsstörungen) müssen bei der Anwendung mit der klinischen Notwendigkeit der Therapie berücksichtigt werden. Die Toxizitätsrisiken können minimiert werden durch Anwendung regelmäßigen Monitorings der Nierenfunktion, des Audiogramms und der Gleichgewichtsprüfung. Im Fall einer Toxizität sollte wegen der möglichen Irreversibilität eine erneute Nutzen- Risiko Analyse vorgenommen und dokumentiert werden, bevor Amikacin weiter angewendet wird. Sollten Nebenwirkungen auftreten, die nicht in der Fachinformation aufgeführt sind, müssen diese gemeldet werden. j) Weitere Besonderheiten: Nutzen und Risiko sollen von einem in der Tuberkulose-Therapie erfahrenen Arzt abgewogen werden, um einen optimalen Einsatz und Sicherheit von Amikacin zu gewährleisten. k) Zustimmung des pharmazeutischen Unternehmers: Die folgenden pharmazeutischen Unternehmer haben für ihre Amikacin-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: B. Braun Melsungen AG und Fresenius Kabi Deutschland GmbH. 2. Anforderungen an eine Verlaufsdokumentation gemäß § 30 Abs. 4 AM-RL: entfällt. 35 XIX. unbesetzt 36 XX. Gabapentin zur Behandlung der Spastik im Rahmen der Multiplen Sklerose 1. Hinweise zur Anwendung von Gabapentin zur Behandlung der Spastik bei Multipler Sklerose gemäß § 30 Abs. 2 AM-RL a) Nicht zugelassenes Anwendungsgebiet (Off-Label-Indikation): Spastik bei Multipler Sklerose, wenn mit den dafür zugelassenen Substanzen bei angemessener Dosierung und Anwendungsdauer keine ausreichende Linderung erzielt werden konnte oder Unverträglichkeit vorliegt. b) Behandlungsziel: Linderung des gesteigerten, spastischen Muskeltonus und Reduktion der Häufigkeit schmerzhafter krampfartiger Muskeltonussteigerungen (Spasmen, Myoklonien). c) Folgende Wirkstoffe sind zur Behandlung der Spastik im Rahmen der Multiplen Sklerose zugelassen: Baclofen (oral), Baclofen (intrathekal), Tizanidinhydrochlorid, Nabiximols - Delta-9-Tetrahydrocannabinol (THC) + Cannabidiol (CBD) d) Spezielle Patientengruppe: Patienten mit Multipler Sklerose und spastischer Bewegungsstörung (Tetra- oder Paraspastik) und/oder schmerzhafte Spasmen. Einschränkungen hinsichtlich anderer Begleiterkrankungen bestehen nach dem aktuellen wissenschaftlichen Erkenntnisstand (10/2012) nicht bzw. entsprechen der aktuellen Fachinformation für die zugelassenen Anwendungsgebiete. e) Nennung der Patienten, die nicht behandelt werden sollen: Gegenanzeigen entsprechen denen der aktuellen Fachinformation für die zugelassenen Anwendungsgebiete. f) Dosierung: Gabapentin soll einschleichend mit einer Einzeldosis von 100 mg 3 bis 4 x täglich ein-dosiert werden. Die Aufdosierung erfolgt nach klinischer Wirkung. Die Wirkdosis liegt zwischen 1200 mg/die (Mueller et al. 1997) und 2700 mg/die (Cutter et al. 2000). g) Behandlungsdauer: Ein responder-Status kann 1 bis 2 Wochen nach Erreichen der Zieldosis von 1200 mg/die beurteilt werden. Die Behandlungsdauer richtet sich nach Erfordernis und klinischer Wirkung auf das Zielsymptom. Die Behandlung ist zu beenden, wenn keine Wirksamkeit erkennbar ist. Gabapentin sollte dabei ausschleichend reduziert werden, da bei plötzlichem Absetzen die Gefahr von Krampfanfällen besteht. h) Wann sollte die Behandlung abgebrochen werden? Die Behandlung muss abgebrochen werden bei Unverträglichkeit (siehe aktuelle Fachinformation für die zugelassenen Anwendungsgebiete). i) Nebenwirkungen/Wechselwirkungen, wenn diese über die der aktuellen Fachinformation für die zugelassenen Indikationen hinausgehen oder dort nicht erwähnt sind: Nach der Berufsordnung der Ärzte sind Verdachtsfälle von Nebenwirkungen, deren Art, Ausmaß oder Ergebnis von den in der Fachinformation genannten abweichen, der Arzneimittelkommission der deutschen Ärzteschaft (AkdÄ) bzw. der zuständigen Bundesoberbehörde zu melden. Dies gilt auch für Arzneimittel, die im Off-Label-Use eingesetzt werden. Auch für Patienten besteht die Möglichkeit, Nebenwirkungen direkt an die Bundesoberbehörden zu melden. j) Zustimmung des pharmazeutischen Unternehmers: 37 Die folgenden pharmazeutischen Unternehmer haben für ihre Gabapentin-haltigen Arzneimittel eine Anerkennung des bestimmungsgemäßen Gebrauchs abgegeben (Haftung des pharmazeutischen Unternehmers), sodass ihre Arzneimittel für die vorgenannte Off-Label-Indikation verordnungsfähig sind: 1 A Pharma GmbH, AAA-Pharma GmbH, AbZ-Pharma GmbH, Actavis Deutschland GmbH & Co. Both the PDE approach and the ‘current approach’ should be equally acceptable. In any case it should not be a requirement for all products manufactured. The PDE-approach may particularly benefit hazardous contaminants such as highly sensitizing materials (such as beta lactams), biological preparations (e.g. from live micro- organisms), certain hormones, cytotoxics and other highly active materials. (see also Chapter 3, no. 3.6) Proposed change (in bold): A Quality Risk Management approach should be used based upon the potential cross contamination risks presented by the products manufactured. This may range from dedicating specific product contact parts to dedication of the entire manufacturing facility. It may be acceptable to confine manufacturing activities to a segregated, self contained production area within a multiproduct facility, where justified. Proposed change: The supply chain and traceability records for each active substance (including active substance starting materials) should be available and be retained by the EEA based manufacturer or importer of the medicinal product. 5.27; 4th paragraph Comment: In exceptional circumstances where direct audit of the active substance manufacturer is not possible, other arrangements for verifying the GMP status of the active substance manufacturer may be deemed acceptable. This could include for exemple remote audit (cf EMA GMP Q&A 3 under GMP PaRT II) and cover the very specific situation of atypical actives. Reasons for changes: Changes have been made to sections 17 to 20 to improve the guidance on prevention of cross-contamination and to refer to toxicological assessment guidance. Changes were also introduced in sections 26 to 28 on the qualification of suppliers in order to reflect the legal obligation of manufacturing authorisation holders to ensure that active substances are produced in accordance with GMP. The changes include supply chain traceability. Containers should be cleaned where necessary and labelled with the prescribed data. 5.4 Damage to containers and any other problem which might adversely affect the quality of a material should be investigated, recorded and reported to the Quality Control Department. 5.5 Incoming materials and finished products should be physically or administratively quarantined immediately after receipt or processing, until they have been released for use or distribution. 5.6 Intermediate and bulk products purchased as such should be handled on receipt as though they were starting materials. 5.7 All materials and products should be stored under the appropriate conditions established by the manufacturer and in an orderly fashion to permit batch segregation and stock rotation. 5.8 Checks on yields, and reconciliation of quantities, should be carried out as necessary to ensure that there are no discrepancies outside acceptable limits. 5.9 Operations on different products should not be carried out simultaneously or consecutively in the same room unless there is no risk of mix-up or cross- contamination. 5.10 At every stage of processing, products and materials should be protected from microbial and other contamination. 5.11 When working with dry materials and products, special precautions should be taken to prevent the generation and dissemination of dust. This applies particularly to the handling of highly active or sensitising materials. 5.12 At all times during processing, all materials, bulk containers, major items of equipment and where appropriate rooms used should be labelled or otherwise identified with an indication of the product or material being processed, its strength (where applicable) and batch number. Where applicable, this indication should also mention the stage of production. 3 5.13 Labels applied to containers, equipment or premises should be clear, unambiguous and in the company’s agreed format. It is often helpful in addition to the wording on the labels to use colours to indicate status (for example, quarantined, accepted, rejected, clean ...). 5.14 Checks should be carried out to ensure that pipelines and other pieces of equipment used for the transportation of products from one area to another are connected in a correct manner. 5.15 Any deviation from instructions or procedures should be avoided as far as possible. If a deviation occurs, it should be approved in writing by a competent person, with the involvement of the Quality Control Department when appropriate. 5.16 Access to production premises should be restricted to authorised personnel. Prevention of cross-contamination in production 5.17 Normally, the production of non-medicinal products should be avoided in areas and with equipment destined for the production of medicinal products but in exceptional circumstances could be allowed where the measures to prevent cross contamination with medicinal products described below and in Chapter 3 can be applied. The production of technical poisons, such as pesticides and herbicides, should not be allowed in premises used for the manufacture of medicinal products. 5.18 Contamination of a starting material or of a product by another material or product must be avoided. This risk of accidental cross-contamination arises from the uncontrolled release of dust, gases, vapours, sprays, genetic material or organisms from active substances, other starting materials, products in process, from residues on equipment, and from operators’ clothing. The significance of this risk varies with the type of contaminant and of product being contaminated. Products in which cross contamination is likely to be most significant are those administered by injection and those given over a long time. Cross contamination should be avoided by robust design of the premises, equipment and processes which take place within a manufacturing facility. This should be supported by appropriate procedures and technical or organizational measures, including reproducible cleaning and decontamination processes of validated effectiveness. 5.19 A toxicological evaluation should be the basis for the establishment of threshold values in relation to the products manufactured (see Guideline on setting health based exposure limits for use in risk identification in the manufacture of different medicinal products in shared facilities). Where the toxicological evaluation supports a threshold value, this should be used as an input parameter in risk assessment. A Quality Risk Management approach should be used based upon this toxicological evaluation and the potential cross contamination risks presented by the products manufactured. Factors including; facility/equipment design, personnel flow, physico-chemical characteristics of the active substance, process characteristics, cleaning processes and analytical capabilities relative to the threshold values for products should also be taken into account. The outcome of the Quality Risk Management process should be the basis for determining the necessity for and extent to which equipment and facilities should be dedicated to a particular 4 product or product family. This may range from dedicating specific product contact parts to dedication of the entire manufacturing facility. It may be acceptable to confine manufacturing activities to a segregated, self contained production area within a multiproduct facility, where justified. 5.20 Technical and organisational measures to mitigate risks of cross-contamination could include, but are not limited to, the following: Technical Measures  dedicated facilities,  self-contained production areas having separate processing equipment and separate HVAC systems. It may also be desirable to isolate certain utilities from those used in other areas.  design of manufacturing process, facility and equipment to minimize opportunities for cross contamination during processing, maintenance and cleaning  use of “closed systems” for processing and material / product transfer between equipment,  use of physical barrier systems, including isolators, as containment measures  Controlled removal of dust close to source of the contaminant e.g. through localised extraction  dedication of processing equipment, dedication of product contact parts or dedication of selected parts which are harder to clean (e.g. filters), dedication of maintenance tools  use of disposable technologies  use of equipment designed for ease of cleaning  appropriate use of air-locks and pressure cascade to confine potential airborne contaminant within a specified area  minimising the risk of contamination caused by recirculation or re-entry of untreated or insufficiently treated air  use of automatic clean in place systems of validated effectiveness,  for common general wash areas, separation of equipment washing, drying and storage areas, Organisational Measures  Dedicating the whole manufacturing facility or a self contained production area on a campaign basis (dedicated by separation in time) followed by a cleaning process of validated effectiveness,  Keeping protective clothing inside areas where products with high risk of cross contamination are processed, 5  Cleaning verification after each product campaign instead of a cleaning validation should be considered as a detectability tool to support effectiveness of the Quality Risk Management approach,  Cleaning of working areas and surfaces followed by execution of a comprehensive sampling protocol for critical surfaces  Use of air samples and wipe/swab samples taken in adjoining areas outside the working area to demonstrate the efficiency of mitigation measures for airborne and mechanical transfer of contaminant,  Specific measures for waste handling, contaminated rinsing water and soiled gowning,  Recording of spills, accidental events or deviations from procedures  Design of cleaning processes for manufacturing equipment and building facilities such that the cleaning processes in themselves do not present a cross contamination risk.  Design of detailed records for cleaning processes to assure completion of cleaning in accordance with approved procedures and use of cleaning status labels on equipment and manufacturing areas,  Use of common general wash areas on a campaign basis.  Monitoring of working behaviour to ensure training effectiveness and compliance with the relevant procedural controls. 5.21 Measures to prevent cross-contamination and their effectiveness should be checked periodically according to set procedures. Validation 5.22 Validation studies should reinforce Good Manufacturing Practice and be conducted in accordance with defined procedures. Results and conclusions should be recorded. 5.23 When any new manufacturing formula or method of preparation is adopted, steps should be taken to demonstrate its suitability for routine processing. The defined process, using the materials and equipment specified, should be shown to yield a product consistently of the required quality. 5.24 Significant amendments to the manufacturing process, including any change in equipment or materials, which may affect product quality and/or the reproducibility of the process, should be validated. 5.25 Processes and procedures should undergo periodic critical re-validation to ensure that they remain capable of achieving the intended results. Starting materials 5.26 The selection, qualification, approval and maintenance of suppliers of starting materials, together with their purchase and acceptance, should be documented as part of the pharmaceutical quality system. The level of supervision should be proportionate to the risks posed by the individual materials, taking account of their source, manufacturing 6 process, supply chain complexity and the final use to which the material is put in the medicinal product. The supporting evidence for each supplier / material approval should be maintained. Staff involved in these activities should have a current knowledge of the suppliers, the supply chain and the associated risks involved. Where possible starting materials should be purchased directly from the manufacturer of the starting material.. The quality requirements established by the manufacturer for the starting materials should be discussed and agreed with the suppliers. Appropriate aspects of the production, and control, including handling, labelling, packaging and distribution requirements, complaints, recalls and rejection procedures should be documented in a quality agreement or specification. 5.27 For the approval and maintenance of suppliers of active substances and excipients, the following is required: Active substances Supply chain traceability should be established and the associated risks, from active substance starting materials to the finished medicinal product, should be formally assessed and periodically verified. Appropriate measures should be put in place to reduce risks to the quality of the active substance. The supply chain and traceability records for each active substance (including active substance starting materials) should be available and be retained by the EEA based manufacturer or importer of the medicinal product. Audits should be carried out at the manufacturers and distributors of active substances to confirm that they comply with the relevant good manufacturing practice and good distribution practice requirements. The holder of the manufacturing authorization shall verify such compliance either by himself or through an entity acting on his behalf under a contract. For veterinary medicinal products, audits should be conducted based on risk. Audits should be of an appropriate duration and scope to ensure that a full and clear assessment of GMP is made; consideration should be given to potential cross- contamination from other materials on site. The report should fully reflect what was done and seen on the audit with any deficiencies clearly identified. Corrective and preventive actions should be implemented. Further audits should be undertaken at intervals defined by the quality risk management process to ensure the maintenance of standards and continued use of the approved supply chain. Excipients Excipients which are considered to pose a particular risk to the quality of the medicinal product, based on formalised quality risk management, should be given similar attention to those for active substances. 5.28 For each delivery of starting material the containers should be checked for integrity of package, including tamper evident seal where relevant, and for correspondence between the delivery note, the supplier’s labels and approved supplier information maintained by the medicinal product manufacturer. The receiving checks on each delivery should be documented. 7 5.29 If one material delivery is made up of different batches, each batch must be considered as separate for sampling, testing and release. 5.30 Starting materials in the storage area should be appropriately labelled (see Chapter 5, item 13). Labels should bear at least the following information: - The designated name of the product and the internal code reference where applicable; a batch number given at receipt; - Where appropriate, the status of the contents (e.g. in quarantine, on test, released, rejected); - Where appropriate, an expiry date or a date beyond which retesting is necessary. When fully computerised storage systems are used, all the above information need not necessarily be in a legible form on the label. 5.31 There should be appropriate procedures or measures to assure the identity of the contents of each container of starting material. Bulk containers from which samples have been drawn should be identified (see Chapter 6, item 13). 5.32 Only starting materials which have been released by the Quality Control Department and which are within their shelf life should be used. 5.33 Manufacturers of finished products are responsible for any testing of starting materials as described in the marketing authorisation dossier. They can utilise partial or full test results from the approved starting material manufacturer but must, as a minimum, perform identification testing of each batch themselves according to annex 8. The rationale for the outsourcing of this testing should be justified and documented and the following requirements should be fulfilled: a) A formal agreement should be signed, according to chapter 7, between the finished product manufacturer and the starting material manufacturer. Among the respective responsibilities described in the formal agreement, special attention should be paid to those related to the distribution conditions (transport, wholesaling, storage and delivery) in order to maintain the quality characteristics of the starting materials and to ensure that test results remain applicable to the delivered material. b) The finished product manufacturer should perform audits at appropriate intervals at the site(s) carrying out the testing (including sampling) of the starting materials in order to assure compliance with Good Manufacturing Practice and with the specifications and testing methods described in the Marketing Authorisation dossier. c) The certificate of analysis provided by the starting material manufacturer should be signed by a designated person with appropriate qualifications and experience. This person should ensure that each batch has been manufactured and checked for compliance with the requirements of the formal agreement. d) The finished product manufacturer should have a significant experience in dealing with the starting material manufacturer including assessment of batches previously received and the history of compliance before reducing in-house testing. Any significant change in the manufacturing or testing processes should be considered. 8 e) The finished product manufacturer should also perform a full analysis at appropriate intervals and compare the results with the supplier’s certificate of analysis in order to check the reliability of the latter. Should this testing identify any discrepancy then an investigation should be performed and appropriate measures taken. The acceptance of certificate of analysis from the supplier should be discontinued until these measures are completed. Notes: 1. A similar approach should apply to packaging materials as stated in GMP part I, 5.41. 2. Identity testing of starting materials should be performed according to the methods and the specifications of the relevant Marketing Authorisation dossier. 5.34 Starting materials should only be dispensed by designated persons, following a written procedure, to ensure that the correct materials are accurately weighed or measured into clean and properly labelled containers. 5.35 Each dispensed material and its weight or volume should be independently checked and the check recorded. 5.36 Materials dispensed for each batch should be kept together and conspicuously labelled as such. Processing operations: intermediate and bulk products 5.37 Before any processing operation is started, steps should be taken to ensure that the work area and equipment are clean and free from any starting materials, products, product residues or documents not required for the current operation. 5.38 Intermediate and bulk products should be kept under appropriate conditions. 5.39 Critical processes should be validated (see "Validation" in this Chapter). 5.40 Any necessary in-process controls and environmental controls should be carried out and recorded. 5.41 Any significant deviation from the expected yield should be recorded and investigated. Packaging materials 5.42 The purchase, handling and control of primary and printed packaging materials shall be accorded attention similar to that given to starting materials. 5.43 Particular attention should be paid to printed materials. They should be stored in adequately secure conditions such as to exclude unauthorised access. Cut labels and other loose printed materials should be stored and transported in separate closed containers so as to avoid mix-ups. Packaging materials should be issued for use only by authorised personnel following an approved and documented procedure. 5.44 Each delivery or batch of printed or primary packaging material should be given a specific reference number or identification mark. 9 5.45 Outdated or obsolete primary packaging material or printed packaging material should be destroyed and this disposal recorded. Packaging operations 5.46 When setting up a programme for the packaging operations, particular attention should be given to minimising the risk of cross-contamination, mix-ups or substitutions. Different products should not be packaged in close proximity unless there is physical segregation. 5.47 Before packaging operations are begun, steps should be taken to ensure that the work area, packaging lines, printing machines and other equipment are clean and free from any products, materials or documents previously used, if these are not required for the current operation. The line-clearance should be performed according to an appropriate check-list. 5.48 The name and batch number of the product being handled should be displayed at each packaging station or line. 5.49 All products and packaging materials to be used should be checked on delivery to the packaging department for quantity, identity and conformity with the Packaging Instructions. 5.50 Containers for filling should be clean before filling. Attention should be given to avoid and remove any contaminants such as glass fragments and metal particles. 5.51 Normally, filling and sealing should be followed as quickly as possible by labelling. If it is not the case, appropriate procedures should be applied to ensure that no mix-ups or mislabelling can occur. 5.52 The correct performance of any printing operation (for example code numbers, expiry dates) to be done separately or in the course of the packaging should be checked and recorded. Attention should be paid to printing by hand which should be re-checked at regular intervals. 5.53 Special care should be taken when using cut-labels and when over-printing is carried out off-line. Roll-feed labels are normally preferable to cut-labels, in helping to avoid mix-ups. 5.54 Checks should be made to ensure that any electronic code readers, label counters or similar devices are operating correctly. 5.55 Printed and embossed information on packaging materials should be distinct and resistant to fading or erasing. 5.56 On-line control of the product during packaging should include at least checking the following: a) General appearance of the packages; b) Whether the packages are complete; 10 c) Whether the correct products and packaging materials are used; d) Whether any over-printing is correct; e) Correct functioning of line monitors. Samples taken away from the packaging line should not be returned. 5.57 Products which have been involved in an unusual event should only be reintroduced into the process after special inspection, investigation and approval by authorised personnel. Detailed record should be kept of this operation. 5.58 Any significant or unusual discrepancy observed during reconciliation of the amount of bulk product and printed packaging materials and the number of units produced should be investigated and satisfactorily accounted for before release. 5.59 Upon completion of a packaging operation, any unused batch-coded packaging materials should be destroyed and the destruction recorded. A documented procedure should be followed if uncoded printed materials are returned to stock. Finished products 5.60 Finished products should be held in quarantine until their final release under conditions established by the manufacturer. 5.61 The evaluation of finished products and documentation which is necessary before release of product for sale are described in Chapter 6 (Quality Control). 5.62 After release, finished products should be stored as usable stock under conditions established by the manufacturer. Rejected, recovered and returned materials 5.63 Rejected materials and products should be clearly marked as such and stored separately in restricted areas. They should either be returned to the suppliers or, where appropriate, reprocessed or destroyed. Whatever action is taken should be approved and recorded by authorised personnel. 5.64 The reprocessing of rejected products should be exceptional. It is only permitted if the quality of the final product is not affected, if the specifications are met and if it is done in accordance with a defined and authorised procedure after evaluation of the risks involved. Record should be kept of the reprocessing. 5.65 The recovery of all or part of earlier batches which conform to the required quality by incorporation into a batch of the same product at a defined stage of manufacture should be authorised beforehand. This recovery should be carried out in accordance with a defined procedure after evaluation of the risks involved, including any possible effect on shelf life. The recovery should be recorded. 11 5.66 The need for additional testing of any finished product which has been reprocessed, or into which a recovered product has been incorporated, should be considered by the Quality Control Department. 5.67 Products returned from the market and which have left the control of the manufacturer should be destroyed unless without doubt their quality is satisfactory; they may be considered for re-sale, re-labelling or recovery in a subsequent batch only after they have been critically assessed by the Quality Control Department in accordance with a written procedure. The nature of the product, any special storage conditions it requires, its condition and history, and the time elapsed since it was issued should all be taken into account in this assessment. Where any doubt arises over the quality of the product, it should not be considered suitable for re-issue or re-use, although basic chemical reprocessing to recover active ingredient may be possible. Any action taken should be appropriately recorded. Product shortage due to manufacturing constraints 5.68 "The holder of a marketing authorisation for a medicinal product should, within the limits of their responsibilities, ensure appropriate and continued supplies of that medicinal product to pharmacies and persons authorised to supply medicinal products 1 . The marketing authorisation holder should be informed in a timely manner in case of any constraints in manufacturing operations which may result in an abnormal restriction in the supply of a medicinal product. The holder should also notify the competent authority if the product ceases to be placed on the market of the Member State, either temporarily or permanently. Such notification shall, otherwise than in exceptional circumstances, be made no less than 2 months before the interruption in the placing on the market of the product 2 1 Article 81 of Directive 2001/83/EC, as amended 2 Article 23a of Directive 2001/83/EC
